How to Obtain the Ayushman Bharat Card in Delhi

1. Check Eligibility: Visit the official PMJAY website and click on the 'Am I Eligible' section. Enter your mobile number and the required details to verify eligibility. 

2. Documentation: Ensure you have the necessary documents, including Aadhaar card, ration card, or any other government-approved identification that verifies your eligibility.

3. Application Process:

   • Online: Eligible individuals can apply online through the official PMJAY portal. After logging in, fill out the application form with the required details and upload the necessary documents.
   • Offline: Visit an Ayushman Bharat kiosk located in empanelled hospitals or Common Service Centers (CSCs) in Delhi. The staff will assist you in verifying your eligibility and completing the application process. 
     
     

4. Card Issuance: Upon successful verification, the Ayushman Bharat card will be generated. Ensure all details are correct and keep the card safely, as it is required to avail of cashless healthcare services under the scheme.
